Attached is the fillable form to participate in the June 10th workshop day for 4-H volunteers and youth.  Please read the form carefully.  Anyone 12 and up is welcome and it should be a fantastic training opportunity! You can meet other 4-H volunteers or members, network and exchange ideas - all while learning vital information to refresh or improve your program.

Conference is open to 4-H adult volunteers and 4-H members ages 12 and up

Please return registration form by Friday, June 2, 2017 to Nancy Wilhelm, State 4-H Office, 1376 Storrs Road, Storrs, CT 06269-4134. Include $10 registration fee for each 4-H participant both adult and youth. Make checks payable to UConn. Health Form required for all youth not accompanied by a parent/guardian.
