Carl,

The most common things I tell people are:

Don't build tiles with ArcGIS Online (unless you have to)
Don't host tiled layers with ArcGIS Online (unless you have to)
Don't make apps public if they have credit consuming widgets
Leverage ArcGIS for Server, if you have it, whenever possible
